Seishiro Tokai (Kazunari Ninomiya) works as a surgeon at a university hospital. He possesses excellent technique with suture and carries a surgery success rate of 100%. Due to his arrogant attitude and personality, he comes into conflict with those around him. A new doctor tries to introduce the latest medical device for surgery, which doesn't need a surgeon's hand. Seishiro Tokai is skeptical of the device. He faces the university hospital administration and reveals corruption and hidden past behind the medical device.

Eijiro Saito is a graduate of the prestigious Eidai Medical School and is sent to Eidai Hospital as a trainee doctor. There, under the supervision of Dr. Shiratori at the surgery department, Eijiro trains hard with Dekune and Munekata. But all they seem to do is take care of menial work for low hours and low pay. So low in fact bad that they have to do part-time work at other hospitals just to make ends meet. Eijiro picks Seido Hospital, run by Dr. Hattori, for his part-time job. The surgery is like a battleground. Eijiro is very impressed by his senior Dr. Ushida and the nurse Kaori. But one night, when he is on duty, Eijiro runs from the surgery leaving a badly injured patient behind! Mecha-Mecha Iketeru! is a popular Japanese variety show, shown on Fuji TV. The hosts of the show are the owarai duo Ninety-Nine, and the show is usually attended by regulars Yoiko, Gokuraku Tombo, Oasiz, Shinji Takeda, Akiko Hinagata, and Sarina Suzuki. However, Gokuraku Tombo's Keiichi Yamamoto has not appeared on the show since being involved in a scandal in 2006. Egashira 2:50 also appears as a semi-regular guest. The show is also known as Mecha-Ike.

The Straw Hats visits an island, known as Mecha Island, where a fisherman sings an old folk song about a Golden Crown. Searching for that mysterious treasure, they find a hidden entrance into the island. The island's leader, Ratchet, impressed with the find and in search of the Golden Crown himself, invites the crew to join him in his search and the crew along with Ratchet and his henchmen enter the cave. As it turns out, the islands true form, is that of a giant turtle. Ratchet, who had known this all along, uses his mechanical castle to take control of the turtle, in order to use it, to take over the world. Now the Straw Hats have to stop not only Ratchet, but also the helpless turtle, from crashing into a nearby island.
